APPLICATION FOR TRANSPORTATION SERVICE FORM

Students are eligible for Yellow School Bus Service or Metrocard Service in accordance with Department of Education policies. The parent and/or legal guardian of each student attending a New York City Public School or approved private school must complete this registration form and return it to the student’s school by the last day of school.

Option 2: Request for MTA Metrocard: As a parent and/or legal guardian of the above noted child/children, I request an MTA Metrocard for (2025-2026) School year.
Option 3: Decline DOE Transportation Service: As a parent /or legal guardian of the above noted child/children, I decline student transportation Services for the (2020 – 2021) school year and understand that I must provide transportation for my student. I understand that I may request Department of Education transportation service at any time during the school year.
