Greene Middle School Parental Consent Form

Dear Parent/Guardian:

Based upon Board Policy #7514 'Extracurricular Activities- Parental Consent Policy' which became effective with the 2003/2004 school year, NO STUDENT may participate in a recognized school club or activity without written parental consent. Please complete this form as soon as possible.

Please note: We will continue to require special parental consent for Inter-Scholastic athletic participation, field trips, or competitions that require school transportation, and some club activities such as Ski Club.
